How It Works

  1. Enter your age. Your age between 20 and 79 years. Age is a non-modifiable factor in cardiovascular risk estimation.
  2. Select your sex. Choose Male or Female as assigned at birth. Sex affects the Framingham risk coefficients used in the calculation.
  3. Enter your systolic blood pressure. Your typical systolic blood pressure in mmHg (70–250). Indicate whether you take blood pressure medication, as the calculator uses different coefficients for treated and untreated values.
  4. Enter your cholesterol values. Total cholesterol and HDL cholesterol in mg/dL or mmol/L. The calculator automatically converts between units using the standard conversion factor (1 mmol/L = 38.67 mg/dL).
  5. Select smoking and diabetes status. Indicate whether you currently smoke and whether you have been diagnosed with diabetes. These are significant risk factors in the Framingham equations.
  6. Calculate your heart age. The calculator computes your 10-year cardiovascular risk using Framingham-derived equations, then estimates the age at which a person with optimal risk factors would have the same risk level.
  7. Review the results. The calculator displays your estimated heart age, the difference from your chronological age, educational interpretation, and the estimated 10-year risk compared with optimal risk.

Formula Used

Estimated Heart Age = Age at which optimal risk profile produces same 10-year CVD risk as user's profile

The heart age calculation uses the Framingham Heart Study 10-year cardiovascular disease risk equations:

10-Year CVD Risk = 1 − S0(10)e(Individual Score − Mean)

For each sex, the Individual Score is computed using the natural logarithm of each risk factor multiplied by its regression coefficient from the Framingham study:

Individual Score = β1 × ln(Age) + β2 × ln(Total Cholesterol) + β3 × ln(HDL) + β4 × ln(SBP) + β5 × Smoking + β6 × Diabetes

The coefficients differ by sex and depend on whether the person is on blood pressure medication (SBP coefficient is different for treated vs. untreated hypertension). The heart age is found by solving the equation:

This methodology is similar to the approach used by the CDC Heart Age Predictor and the NHS Health Check program. The heart age is rounded to the nearest whole year. If the calculated heart age falls outside the 20–79 range, it is capped at the boundary.

How to Interpret Your Results

The calculator interprets your heart age estimate as follows:

  • Heart Age Lower than Chronological Age (by 3+ years) — Your estimated heart age is lower than your chronological age, suggesting a favorable cardiovascular risk profile relative to your age group. This estimate is based on the combination of risk factors you entered and is provided for reference purposes only.
  • Heart Age Similar to Chronological Age (within ±2 years) — Your estimated heart age is comparable to your chronological age, suggesting your cardiovascular risk profile is broadly consistent with expectations for your age group.
  • Heart Age Moderately Higher (by 3–7 years) — Your estimated heart age is moderately higher than your chronological age, suggesting an elevated cardiovascular risk profile relative to your age group. Modifiable risk factors such as blood pressure, cholesterol, and smoking may be contributing factors.
  • Heart Age Considerably Higher (by 8–14 years) — Your estimated heart age is considerably higher than your chronological age, suggesting a substantially elevated cardiovascular risk profile. Discuss your complete cardiovascular health assessment with your healthcare provider.
